The family of a man who died after eating a McDonald’s Quarter Pounder has now spoken out after learning that he was the first fatality linked to a multi-state E. coli outbreak. James Charles Smith Jr, who goes by JC, was a frequent visitor of the Golden Arches near the Grand Junction, Colorado, home he shared with his wife, Doris.

Quarter Pounders without onions will return to Colorado, Kansas, Utah and Wyoming, where the burgers were removed from all McDonald’s restaurants. The menu item will also return to restaurants in parts of Idaho, Iowa, Missouri, Montana, Nebraska, Nevada, New Mexico and Oklahoma.

"The decision to do this is not one we take lightly, and it was made in close consultation with the CDC," McDonald's USA president Joe Erlinger said in a video statement on Tuesday. McDonald's North America Chief Supply Chain Officer Cesar Piña said in a statement posted Tuesday that an initial investigation into the outbreak indicated "a subset of illnesses may be linked to slivered onions used in the Quarter Pounder and sourced by a single supplier that serves three distribution centers.
